This case details the restructuring a health care delivery system, UnityPoint Health- Fort Dodge, into a Beginner Accountable Care Organization (ACO) after passing health reforms in US. The case discusses in length about how the hospital CEO and staff designed and implemented new models of care delivery and built relationships across health care delivery settings in an attempt to lower health care costs and better coordinate patient care.
Using patient narratives, care delivery prior to the creation of the ACO is compared to care delivery after the formation of the ACO.
