A recent report by the Institute of Medicine has focused again on the frequency of medical errors in the health care industry. Despite the relatively large number of studies on how organizations can work in a very reliable way, and thus to avoid such mistakes, little work has been done in the area of health. This article discusses the ways in which the health care industry is not done systematically in order to achieve high standards of reliability, based in part on two existing theories on the management of high-risk environments - high reliability organization theory (HROT) and normal accident theory (NAT).
