District X, one of about 50 districts in 9 states in South Africa, was a mostly rural region saddled with elevated levels of poverty, poor infrastructure, and high mortality rates; medical needs were high but nearly half of the places in health department were occupied, and staff turnover was high.
There was a push in South Africa to decentralize management to local levels in order to respond to local needs more efficiently and to boost primary care, but the procedures for preparation, reporting and budgeting were exceptionally complicated and did not seem to support that goal. Mr. J wondered what added skills were required for the system to work, and also system itself could be enhanced.
