These circumstances are mostly affected by the distribution of money, power, and resources at national, global and local levels. Therefore, by assessing these social determinants, we can understand the factors which can affect the health of different individuals. Mainly there are five social determinants neighborhood and built environment, health and health Care, social and community context, economic stability and education.

Under neighborhood and built environment, one can ask the question about the residency of the patient like where the patient lives? This will help in assessing the impact of neighborhood environment on the health of the patient. Such that either the patient have immediate access towards the health care and how much polluted the surrounding is in which the patient lives this will help in analyzing the factors which can affect over the health of the patient.

Moreover, under health and healthcare one can ask a question about the accessibility to the healthcare centers like how far the healthcare center is from the residency place? This will help in assessing that how immediately a patient can take services from the healthcare center in an emergency situation. And either the available healthcare centers provides adequate services or not?

Under social and community context one can ask questions about the favorite personality of the patients from their surroundings. It is the fact that some people feel comfortable in talking with their favorite one therefore, after knowing the favorite personality of the patient the nurse can obtain more information about the patient from the relevant person. Furthermore, we can also ask questions about the friend list of the patient and about the social activities in which it take participate this will help in ascertaining the social context of the patient.

Under economic stability determinants, we can ask question about sources of income of the patient and their dependents. This will help in determining the patient’s income hence by understanding the income position of the patient we can conclude that either it has an access towards the resources such as healthy food, housing, effective healthcare and etc.

Under education determinants, we can ask questions about the education background of the patients and their families. Education is a social determinant which is directly linked with other determinants like employment of a person is based upon the education of that person. Hence we can conclude that a highly educated person may have an attract job and hence may a healthy income and in result he is able to afford the healthy foods and effective healthcare service.
